Teeth Grinding:Effects and treatment

                            Have you ever told you grind teeth at nights even without your knowledge?
Do you wake up with pain in the jaws?Do you have tooth worn?or loose ?or broken?IF so, you have teeth grinding or teeth clenching which in medical terms is called Bruxism.Mostly Bruxism is an unconscious habit , you clench your teeth or grind unintentionally at inappropriate times. Bruxers usually don't realize until enough damage has already been done to teeth and jaw.
                            The common cause for bruxism is stress and anxiety.Other cause is abnormal alignment of upper and lower teeth .

                          Stress counseling,meditation,exercise,visiting a therapist help in managing stress
                          Prescribing muscle relaxants and treating the cause will reduce the pain
                          Avoiding alcohol, caffeine,chewing gum and relaxing your face often is recommended.
                          Most common treatment is the use of occlusal splint known as mouth guard or night guard.This will protect the teeth from wearing off

                          Correction of mal-aligned teeth by relieving high or stress points.
                          The use of mouth guard is a conventional treatment for bruxism.The recent advancement in use of Botox injection.Botox injection commonly used with cosmetic procedure now aid in keeping bruxism under control.Although mouth guards protect the teeth successfully from damage during nights,they fail stopping the painful effects of tooth grinding unlike botox.

                           Botox is injected to the largest muscle that is associated with the movement of the jaw.This will weaken the muscle enough to stop the involuntary teeth grinding and thus significantly reduces the damaging effect on teeth and jaw.Need not worry about the voluntary movments such as chewing and smiling as botox dont affect them.Botox don't serve complete cure for bruxism but effective than nightguards in reducing the uncomfortable symptoms.The botox shot given renders its help for 3-4 months.

                                       BAD BREATH

              Bad breath can be a result of your poor oral hygiene or it may be an indication of your other heath problems.Most bad breath starts in your mouth.Bad breath or Halitosis can be embarrassing but it is quite a common problem and has possible number of causes which can easily be prevented or treated.The possible causes for bad breath are

  •    Poor oral hygiene-food particles stuck is a attraction site for bacterial growth resulting in decay and gum disease.Uneven surface of tongue harbors bacteria
  •   Eating food with strong odor-garlic and onions.As you digest these food they enter the blood stream and to lungs.The odor will not completely go until the food is eliminated from the body
  •   Strong smelling drinks such as coffee and alcohol can cause bad breath.
  •   Smoking or chewing tobacco related products
  •   Mouth infections like impacted teeth
  •   Nose,throat infections 
  •   Acid reflex 
  •   Ill-fitting denture
  •   Dry mouth- resulting from decreased flow of saliva.Saliva cleanses the mouth and removes particles that may cause odor.

Check your bad breath:
            Having bad breath is bad enough but not knowing that you do is even worse.Testing yourself is difficult because the location your opening to nose from mouth and also the nose receptor cells get used to the smell.One can't literally smell their own breath.When people used to rub their noses and step back while you are talking,you got your clue.But there are many other ways of finding it out which is as follows

  • Ask someone:First way is an obvious one.Just ask someone else what they think .You can choose a significant other to get this job done.Or a Dentist in your next appointment.

  • Wrist test:Some people check for bad breath by cupping their hands to the nose.But it's just the smell of your hands.Inside of your wrist is not constantly handling things and so Lick the inside of your wrist and let it dry for few seconds.This gives the true reading of your breath.

  • Bad taste:Another way is if you have bad sure have bad breath too.

  • Cotton test:Use the cotton gauze and wipe the back of your tongue and sniff it.If it smells bad ,your breath does.if the saliva gunk in the cotton gauze is of yellowish shade ,it implies than you have breath problems.

  • Floss test:Use Dental floss between the teeth where usually food gets stuck and the smells of the floss is the smell of your breath.

  • Spoon test:Use a spoon to scrape out the back of your tongue.Inspect the saliva gunk ,the darker the color the thicker is the saliva and so does the intensity of smell.

Fight bad breath:
        The bad smell from your mouth is due to volatile sulfur compounds , the product of bacteria over food.Visit a dentist.Most of our effort lie in fighting against the bacterial build up.We inspect for the cause in the mouth like deposits,gum problems,decay,impacted teeth , dry mouth and so on and treat it.If the smell is not due to mouth you are generally referred to physician to check for other problems like respiratory infections ,diabetes,gastric ulcers and so on. 
  • Brush twice daily
  • Floss everyday
  • Clean your tongue which is the main source of bad smell.It is the most important of all.
  • Quit smoking and tobacco related products
  • Drink a lot of water.keep your mouth hydrated
  • Remove your dentures at night and Clean your dentures every morning 
  • Use natural mouth fresheners like mint,anise seeds or fennel and parsley.
  • Be aware of odorous food.Avoid drinking coffee and alcohol
  • Get regular dental check-ups

         As a parent you might be wondering how to keep you child away from oral problems.Babies are born with teeth hidden inside the gums.Teeth shows up their head out of the gums from 6 months on-wards.Teeth that comes out are healthy only when the gums are healthy.Thus, it is important to maintain good oral hygiene practice even before tooth comes out.At the age of 3, Kids have all their baby teeth erupted.Its starts falling out from the age of 6.Most permanent teeth come out by the age of 12-13.Gaps between baby teeth are normal as they make room for permanent teeth.Make your child Tooth-tastic by the following ways....!

0-2 years:
      Age for bottle feeding.Leaving the baby sleep at nights with the bottle will lead to a serious problem of decay in all the baby teeth.This is called Nursing bottle decay. so,
#Avoid bottle feeding. Even if, wipe the gum pad and teeth with clean damp cloth after feeding.
#Brush with soft bristles twice daily
#visit your family dentist at this stage to screen.

3-5 years:
    Age of all baby teeth.
#Make brushing enthusiastic.Using pea-sized amount of Fluoride tooth paste ,brush twice day.Early morning and right before bed.Make sure they don't swallow the paste.

                  Have your dentist informed you might need a root canal treatment?And are you going to have your first root canal treatment and you are quite nervous about that?Here,learn how root canal treatment is been done step by step.
                   Underneath the white enamel and hardest layer dentin of your teeth lies the PULP-The soft tissue that keeps the tooth alive having blood vessels and nerves.Once the decay or fracture approximates or touches the pulp portion ,usually the alarm rings-THE PAINNNNN !!!A tooth can be divided into crown- that portion of the tooth you can see above the gums and root - that portion of teeth that is beneath the gums and you cannot see.The pulp starts from middle of the crown and it stretches inside the canal running till the full length of the root. Once the decay/crack/fracture touches from the crown portion or elsewhere close to the canal... the entire pulp inside the canal get infected leading to its death.Thus root canal treatment(RCT) serves in removing the infected pulp,disinfecting the canal in the root where the pulp was in ,finally filling and sealing it.

  • Step 1:Opening for access to root- Pre-op x-ray will be taken.A shot of local anesthesia will to given to make the tooth numb.An opening will made in the crown of the tooth to gain access to the infected pulp.

  • Step 2:Removing infected pulp- Specially designed small sized instruments are used to remove the infected pulp and to clean the canal.No pain will be there during this as the tooth is numb.Once the dead or dying pulp tissue containing nerves is removed, the tooth becomes pain free.The canal is shaped to receive filling.The canal are then and there irrigated with saline and disinfectant solutions.

  • Step 3:Filling the canal- Temporary filling may be placed for observation.Once complete cleaning and shaping done fillers (gutta-percha usually) are compressed into the canal and sealed off with heat.Sealing is done to ensure the canal area becomes free from contamination and infection.

  • Step 4:Rebuilding the tooth-Temporary or permanent filling will be placed in the opening for access.Tooth will be rebuild to its actual shape.In case severe damage by decay or fracture the dentist may place a post extending from canal around which the tooth structure will be built.The entire procedure can be completed of in single sitting in about an hour.

  • Step 5:Cap placement- The root canal treatment is completed with the tooth structure rebuilt.But a cap over the treated tooth is a MUST to completely seal off and for strength ,restoring normal tooth function such as effective chewing.The tooth is prepared for cap in the day of first sitting and cap can be cemented in 2-3 days.Antibiotics will be given.

                  The common problem everybody surfing right now in the internet is pain in the back of mouth with difficulty in mouth opening.Probably ,it is Impacted wisdom tooth- a common,usually painful problem people at 3rd decade of life are facing.Understanding the sings of impacted wisdom tooth can help you seek the dentist at right time.

                  Wisdom tooth is  the last one to emerge out of gums in back of your mouth both in upper and lower arch.Their development begin at any time of teen age they fully emerge between the age of 17 -25 years of age.

                     Wisdom tooth that don't erupt completely at early 20's are called impacted.As they are the last teeth to erupt ,the jaw don't usually have enough room to accommodate them.They get trapped inside the jaw or gums.They can be fully or partially impacted.Patients with completely impacted teeth will experience discomfort and other teeth can be knocked out of alignment as impacted teeth exhibit force against the gums by pushing to come out.Partially impacted teeth can also cause enough damage to the neighboring teeth by harboring food particles leading to infection and decay.It is believed that this generation is facing more of this impaction trouble as there is decrease in jaw size due to soft diet(evolution).Impacted teeth can be identified in regular dental check-ups too.

There is a connection between Allergies and Tooth ache:
Allergy related tooth ache can present as an ache, sensitivity to hot or cold, pain even in change of position like sitting or standing. It usually happens in the upper back teeth.

What happens is that it when your body's immune system gears up when you catch a cold mucous fills up the sinus cavities which are above the roots of the your upper back teeth these cavities which are supposed to be empty when clogged with mucous secretions, irritate the teeth and cause pressure pain. This occurs in case if there is no decay of any sort in the teeth and it is purely sinusitis which vanishes when you take meds for cold.

                     Smoking kills the gums to begin with....

    One more reason that is gearing up to say NO SMOKING is that you having to move around tooth less...!

Yes,smoking can cause gum problems and loosening of tooth..... and here is HOW..!

Disease masking:                     
                Smokers usually have four fold increased risk for gum problems than non-smokers. Smokers don't usually have pronounced swelling ,redness and bleeding from gums which generally are the marker for the gum disease in non-smoking people-the gingivitis stage.This is because the nicotine present in cigarettes shrinks the blood vessels and weaken that basic response.The alarm rings only the when.. gum disease has progressed to bone level,leaving you with late stage symptoms such as receding gums,bone loss,loosening of gum grip forming pockets between your tooth and its gum-the periodontitis stage.This is called disease masking.Turning up to a dentist at the early bleeding gums stage can put your teeth at better salvation where as later stage treatment compromises... smokers may not have the former choice !!!
